PROGRAM APPLICATION INSTRUCTIONS for 2008

| Dear Property Owner: |

| Thank you for your interest in the Private Participation Concrete Replacement Program offered by the Village of Elk Grove Village. The contractor performing the concrete work for this year's program is RAI Concrete of West Chicago, Illinois. |

The PPCRP program allows residents to replace their driveway apron, and abutting curb and sidewalk within the public right-of-way. Driveways, walkways, patios and other concrete work located on private property are not included in the program however; the property owner may contract for additional work directly with RAI Concrete. This additional work would require a separate permit from the Engineering Department. The following steps should be followed to participate in the program. |

Step 1: |
The property owner must complete the enclosed Program Application Form and return it to the Public Works Department, located at 666 Landmeier Road, as soon as possible, but no later than, May 2, 2008 . Completion of an application is not an obligation to participate in the Program. |

Step 2: |
T he Village will visit your property, measure the requested work, and calculate a cost based on contract unit bid prices received from RAI Concrete. |

Step 3: |
The Village will invoice the property owner for the cost of the requested concrete work. A driveway permit application will be included for your use and the $25.00 permit fee will be added to your invoice. |

Step 4: |
If you wish to proceed with the work and are satisfied with the cost estimate, please submit full payment and the completed driveway permit application to the Village Finance Department at 901 Wellington Avenue. Payment must be received no later than June 6, 2008. |

Step 5: |
Upon receipt of payment, driveway work will be scheduled to take place June through July 2008. You will be notified prior to the start of work in your area. Construction scheduling is weather dependent and special date requests cannot be accommodated. Cancellation of work will not be accepted and payment is non-refundable. |

The role of the Village is to serve as an intermediary between you and the contractor. There are no contractor or Village warranties under this Program . However, as with all other work done within the Village, the work performed and the materials used by the contractor will be inspected by Village personnel to insure compliance with Village codes and material standards. |

Can I replace only my curb to eliminate the ‘bump' at the end of my driveway? |
Typically, construction of a new depressed curb along the entire length of the driveway and replacement of the driveway apron is required to eliminate the ‘bump' at the end of the driveway. |

Will the Village pay 50 percent of the concrete work? |
No, the property owner is responsible for 100 percent of the cost of the requested work. The Village oversees the administration and material testing for the concrete work. |

What is the cost for the concrete work? |
The Village will measure the requested work and provide you with a cost estimate based on contract unit bid prices. The following information is provided as a general guide for expected replacement costs. |

Can my driveway apron have a decorative finish? |
No, driveway aprons by Village code must be concrete and special finishes are not allowed. |

Can I widen my driveway apron width? |
Generally, driveway aprons must match the width of the existing driveway and may not exceed 20 feet. If you wish to vary from this policy you would need to obtain the necessary approvals from the Engineering Department prior to submittal of the application. |

I would like my driveway completed prior to a specific date? |
| The contractor has established unit prices based on completing all driveways within one section of the Village at a time. This provides for a more efficient operation and results in lower contract unit costs. Therefore, it is not feasible to honor specific time requests for concrete work . The contractor will schedule work to take place June through July. You will be notified prior to the start of work in your area. Please note that once payment has been received cancellations will not be accepted. |

Will the contractor replace my driveway on private property? |
Typically, the contractor will be willing to contract directly with the property owner for additional concrete work on private property including driveways, walkways and/or patios. Accepting the additional work is at the contractor's option, and the contract unit costs shown above may not apply. The Public Works Department will not oversee any work on private property. Property owners must obtain a separate permit from the Engineering Department for the additional work. |
